The employer did not have a competent person at the construction site conducting frequent and periodic safety inspections at the construction site.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied:  Brandon Hastings dba Hastings CCI Equipment, Inc. had employees exposed to 13 ft., 5 in. fall hazards, while working from a supported scaffold lifted by a rough terrain forklift, on a commercial construction site in Sidney, Illinois.   The employer did not secure the supported scaffold to the rough terrain forklift by a chain or some other secure method.
