29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards created by point of operation, in-going nip points, rotating parts, flying chips and sparks. This violation was observed on or about December 3, 2014, in the machine shop where employees were exposed to rotating parts when operating a lathe with a broken guard. Graham Packaging Company, L.P. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 315463000, citation number 1, item number 1 and was affirmed as a final order on July 21, 2011, with respect to a workplace located at 8966 Latty Ave, Berkeley, MO 63134. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the lathe is guarded to protect employees from the hazards created by an partially guarded chuck and bit.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down isolating, blocking and securing machines or equipment to control hazardous energy. The employer does not ensure that the energy control procedure clearly and specifically outlines the steps for shutting down the trimmer machine. This violation was observed on or about December 03, 2014, at the facility where employees were potentially exposed to hazardous energy sources when servicing the trimmer machine. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard by ensuring that the energy control procedure clearly and specifically outlines the steps for shutting down the trimmer machine.

This violation was observed on or about December 03, 2014, at Trimmer Line#1 where an employee was exposed to caught by hazards when changing bearings on a tipless cutter in that the name of the employee applying the lock was not indicated on it. Pursuant to 29 C.F.R. 1903.19, within ten(10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that lockout devices and tagout devices indicate the identity of the employee applying the device(s).
