29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    a.  On or about December 4, 2014 - at the above addressed jobsite, employees working in a Genie Z-45/25J telescopic boom aerial lift were not attached to the boom or basket by the use of a lanyard, thereby exposing the employees to the hazards associated with a fall of approximately twenty two (22) feet.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    a.  On or about December 4, 2014 - at the above addressed jobsite, the employer and the employee who were both using the Genie Z-45/25J Telescopic Boom Aerial Lift truck were not trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employer and the employee to the hazards associated with falls.
