OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from an elevated platform:    On or about November 21, 2014, employees were exposed to a six to eight foot fall hazard while working from a packaging tote elevated by a powered industrial truck and the employer did not ensure an approved platform was used to elevate employees.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
