29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a. 121 Golden Hills Drive, Dahlonega, GA: Rear of Bldg. No. 10 (Golden Hills Apartments) - Employees (HVAC subcontractors) were in the process of installing soffit air vents on the roof overhang while utilizing a pump jack working at the edge, the employer (General Contractor) did not ensure that fall protection were used by the employees while exposed to fall hazards of approximately 15.9-90 feet, on or about 12/4/14.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: 121 Golden Hills Drive, Dahlonega, (Golden Hills Apartments) - Employees (G.C and subcontractors) were exposed to tripping and/or stepping into hole hazard. Employees were walking and working in close proximity to several large sewer drains/man holes. The hole was used as a run-off for rain water; it was partially covered and not marked and/or identified as to its description, on or about 12/04/14." a. Road Entrance on the right side of the street - sewer drain/man hole measuring 3'6" wide x 4' deep. b. Road Entrance on the left side of the street - sewer drain/man hole measuring 3'6" wide x 4'7" deep.
