29 CFR 1926.250(a)(1): All materials stored in tiers were not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling, or collapse: a) Wall panels that arrived to the construction site on a flatbed truck were left standing vertical after the tie-down straps were removed from one side. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Employees were exposed to 30 ft fall hazard at the 3fl deck where there was a gap in the fall protection cables; cables did not extend all the way to the wall leaving a two ft wide gap. Location: 1151 Bedford Ave, Brooklyn N.Y. On or about 12/2/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
