29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level: Jobsite - Production Area, Line 4 (Extruder/Vacuum): Employees in the production area, whose job tasks include cleaning dies and performing preventative maintenance on the extruder and vacuum areas, are exposed to illness in that the employer is failing to conduct initial monitoring to determine employee exposure to lead. Lead was discovered on wipe samples collected on December 3, 2014 in the production area, Line 4 extruder (15.39 micrograms) and Line 4 vacuum (874.95 micrograms). Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

Yet, the employer is failing to inform affected employees of the content of Appendices A and B of the lead standard. Lead was discovered on wipe samples collected on December 3, 2014 in the production area, Line 4 extruder (15.39 micrograms) and Line 4 vacuum (874.95 micrograms). Jobsite - Maintenance Shop: Maintenance employees performing welding projects using stainless steel are exposed to illness in that the employer is failing to conduct initial monitoring to determine employee exposure to Chromium (VI). Chromium (VI) was detected through the use of ChemTest kits for hexavalent chromium made by MicroTeq. Wipe tests were performed a job-made stainless steel tray welded by a maintenance employee on December 3rd, 2014. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Jobsite - Maintenance Shop: Maintenance employees performing welding projects using stainless steel are exposed to Chromium (VI) and the employer is failing to provide affected employees training to recognize, evaluate and control exposure to hazardous chemicals including, but not limited to, Hexavalent Chromium. Chromium (VI) was detected through the use of ChemTest kits for Hexavalent Chromium made by MicroTeq.
