29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.      At this residential construction site, employees were installing trusses 10 feet above ground level.  The employees were not  protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  At this construction site, debris was not cleared for work areas, passageways in around a ladder which was unserviceable near stairs and the apartment building(located in the center of site and electric meter northeast of the street) to prevent protruding nails which could cause a trip hazard or bodily injury.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.   At this residential construction site, the ladder was defective.  The employees were not  protected from falling by the use of  the ladder because it was taped at the bottom or defective.  The superintendent immediately removed from the work area and discontinued use or withdrawn from service.
