Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) On or about December 4, 2014 - at a jobsite located at 63rd & Martin L. King Drive, Chicago, IL. - employees using a jack hammer to break up the concrete street were not using appropriate personal protective equipment, including, but not limited to face shield, glasses/goggles, ear plugs or respiratory devices, thereby exposing employees to safety and health hazards.

King Drive, Chicago, IL.- employees conducting concrete removal operations in the intersection of 63rd & Martin L. King Drive where signs indicating workers ahead were not installed at the point of hazard, thereby exposing the employees to being struck by vehicular traffic.
