29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive initial training on fork and attachment adaptation, operation, and use limitations: On or about December 1, 2014, at the above addressed jobsite, two employees were exposed to a fall hazard when the operator of the Hyster Fortis 60 forklift attached the Vestil WP-4848 man cage to the forks of the forklift without using the locking pin or the restrain strap that came with the cage.
