29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  On or about December 03, 2014, employee were exposed to struck-by hazards within the machine maintenance shop while the following fixed items of industrial equipment were found not to be securely anchored in order to prevent inadvertent or unforeseen walking or movement of the machinery:  a) Along the center area of the machine maintenance shop a 2 Bridgeport milling machines were not securely anchored.  b) Along the center area of the machine maintenance shop a Seedoorf Acme spot welding machine model 8280, S.N. R101910 was not securely anchored.  c) Along the center area of the welding shop a Jet drill press model 354221, S.N. 13010933 was not securely anchored.  d) Along the center area of the welding shop an Acme spot welding machine type PTOP-6-50, S.N. 1G173  was not securely anchored.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:  Along the welding work area, on or about December 03, 2014, an welding employee was exposed to the hazards of struck-by while working around approximately 17 compressed gas cylinders containing chemicals such as oxygen and acetylene that were not secured and stored.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Along the production floor area, on or about December 03, 2014, and at times prior thereto, employees were exposed to electrical shock hazards of 120 volts from unused openings along the bottom and sides of the electrical distribution panel boxes that were not effectively closed:  a) Along the east side wall of the production floor power enclosure no. AW59541 controlling the electrical power to the AEC plastic molding machine.  b) Along the center north west side of the production, the fixed power enclosure Siemens model E2396 controlling the electrical power various outlets, lighting units and a Mazak molding machine.  c) Along the center side wall of the production next to the welding area, the fixed power enclosure Square D model QOC42US controlling the electrical power to the lighting fixtures, outlets, and Acme spot welding machines.  d) Along the north side wall of the production the fixed power enclosure GE model 72938 controlling the electrical power to the cooler gfi, roof gfi, forklift charger, night lighting, electrical outlets a Tapmatic model SPD7 and a Sear Craftsman 15" drilling machine.  e) Along the south side wall next to the welding area the fixed power enclosure Square D model QOC42US controlling the electrical power to the lighting fixtures, outlets, wall tumble and plaining machine.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Along the production floor, on or about December 03, 2014, the following electrical panel enclosures were found to be covered or partly covered exposing the employees, in the event of an emergency, to limited or no access to the electrical panel disconnects or breaker panels:  a) A General Electric panel enclosure was located along the north wall of the production floor identified to control the main switch for a mold machine labeled TM-2.  b) A Square D panel enclosure model QOC24US located along the center area of the production floor labeled to control work tables 1, 2 and 3.
