29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:   (a) On 12/4/15, an employee was  working below the roof eaves and picking up discarded shingles and exposed to falling materials and tools.   The employee was not wearing a hardhat, nor were employees prevented from being in the work area.  No abatement certification or documentation required.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  On 12/4/15, employee(s) were performing roofing activities and working on 6/12 pitched roof without fall protection provided. These employees were exposed to an 18 foot fall to the pavement below from the commercial roof.  No Abatement Certification or Documentation Required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On 12/4/15, the employer did not provide training to employees to recognize the hazards of the fall to which they were exposed and the correct procedures to follow to prevent a fall.  Abatement certification required within 10 days after the abatement date.  The certification shall include a statement that the abatement is complete, date and method of abatement, and state that employees and their representatives were informed of this abatement.
