(a) The employer ffailed to provide documentation demonstrating the abatement method and completion for Cit 1, Items 1 and 2a issued on 3/16/2015: a) A competent person was not on site to perform frequent and regular inspections of the job site to ensure that hazardous conditions inspected for and corrected when found. b) Employees conducting roofing activities on a roof with a 12:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 19.7 feet.
