29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) On or about December 4, 2014; an employee was working on the Hwy 41 bridge above College Ave. to remove temporary traffic control devices on the northbound bridge and southbound bridge without adequate fall protection. The employee fell approximately 20'-6" through an opening (approximately 13.5") when crossing between the bridge decks landing on concrete and stone below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about December 4, 2014; employee(s) of Mega Rentals, Inc. were working from the bridge surface of highway 41 over College Ave without adequate fall protection. Employees relied on the approximate 32" permanent bridge wall as a guardrail which was not the required 42" (+/- 3" allowance). This exposed employees to fall hazards of approximately 20'-6".
