Ground-fault circuit interrupter (GFCI) protection was not utilized. The generator was not equipped with a ground-fault circuit interrupter (GFCI), nor was an in-line GFCI used in conjunction with the extension cord and flexible electrical cords.  In addition, an assured equipment grounding program (AEGP) was not being utilized as an alternate means of protection.
