29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    A wooden pallet was used as a scaffold platform and was set on the forks on a JCB - 507-42 fork loader.  The employees stood on the scaffold platform on the forks while it was moved horizontally to reach working height.  This exposed employees to the hazard of falling 15 feet.

The scaffold platform was set on the forks of a forklift and used to raise employees to the upper working area.
