29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Jobsite, 680 Pellis Road, Greensburg, PA:  On or about 12/04/14 employees performing roof activities on a roof with a pitch of 4/12 were not protected from falling approximately 16-18 feet to the ground.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a) Jobsite, 680 Pellis Road, Greensburg, PA:  On or about 12/04/14 the employer did not prepare a written training certification record to identify the name, date of employees trained or the signature of the person performing the training.  The employer had purchased fall protection equipment and had it on site but did not certify that he had instructed the employees on its use.
