Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Jobsite, The Crossing at South Fayette, Bridgeville, PA:  On or about 12/04/14 employees engaged in roofing activities on a low-sloped roof were not protected from falling approximately 16-19 feet to lower levels.  There were no guardrail systems, safety net systems, personal fall arrest systems or safety monitoring with warning line systems in use to protect employees from falling.
