29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:       a) 1111 Central Avenue, Westfield, NJ 07090. The employer allowed his employee to operate and fuel a gas powered air compressor when he did not provide a fire extinguisher rated not less than 10B exposing the employees to burn hazards in the event of a fire.       Condition occurred on or about 12/04/14.

The employer did not ensure that the materials hoist was used in a manner specified by its manufacturer. Employees used materials hoist as a ladder to access the roof, thereby exposing employees to fall hazards.      Condition occurred on or about 12/04/14.
