29 CFR 1910.138(a): Appropriate hand protection was not used when employee's hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.    On or about 5 December 2014, and at times prior thereto, 6 employees use kitchen knives to prepare food without the use of hand protection, exposing the workers to an amputation hazard.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.    On or about 5 December 2014, and at times prior thereto, a Robot Coupe food processor being utilized in the kitchen area had cord damage at two separate locations, exposing the employees using it to electrical hazards.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910.     On or about 5 December 2014, and at times prior thereto, electrical breaker panels in the kitchen area were accessed to turn off or on certain breakers in which the area in front of them did not allow for at least 36 inches of clearance, exposing the employees to an electrical hazard.

On 5 December 2014, and at times prior thereto, electrical breaker panels in the kitchen area had cardboard boxes and other items stored underneath or on top of 120 volt electrical breaker panels, exposing the employees to a tripping or fire hazard.
