29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  (a) 526 Halyard Drive - The employer did not ensure that the working level of the scaffold was fully planked between the uprights. An employee worked on the fourth buck and was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.  No abatement certification or documentation required.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  (a) 526 Halyard Drive - The employer did not ensure that walkways were at least 18 inches wide. An employee used a walkway between two sections of scaffolding that was only 12 inches wide on the fourth buck and was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.   No abatement certification or documentation required.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a) 526 Halyard Drive - An employee worked on a scaffolding system four bucks high laying the stone facade on the front of the house. Two of the four legs of the scaffolding did not have base plates installed.  The employee was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.  No abatement certification or documentation required.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  (a) 526 Halyard Drive - The employer did not ensure that the working level of the scaffold had guardrails along the back. The scaffold had cross braces installed which serve as a top rail but there was no midrail installed between the uprights. An employee worked on the fourth buck and was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.  (b) 526 Halyard Drive - The employer did not ensure that the working level of the scaffold had guardrails along the open west side of the outrigger working area. An employee worked on the fourth buck and was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.  No abatement certification or documentation required.

(a) 526 Halyard Drive - The employer did not ensure that the four buck high scaffolding system was braced to the building at the 20 foot height to prevent tipping. The scaffold was end frames were each five feet tall and five feet wide. An employee worked on the fourth buck and was exposed to a fall of approximately 20 feet to the ground on or about December 4, 2014.  No abatement certification or documentation required.
