29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards. On 11/20/14, employees were exposed to fall hazards up to 13 feet as they are required to access road tractor trailer platforms that have either inadequate or no guardrails and at times the load itself without fall protection. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation. On or about 11/20/14, at the Maysville job site, the employer did not ensure injuries recorded were completed in the detail as required by the regulation. i.e. Column D and E had no information, Column F descriptions were not complete and column K was not properly marked indicating the number of days employees had away from work. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
