29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a) Worksite, Eastbound Sunrise Hwy. Service Rd., E. Patchogue,, NY - An employee hanging a sign on a fixed roadside billboard was working from an aerial work platform hoisted by a National Crane, Model #900-A, Serial #293213 approximately 16 feet above the ground and was not wearing a body belt or harness anchored to the boom or basket; on or about 12/4/14.   Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
