A Dayton bench grinder was used with a gap of approximately one-half inch between the abrasive wheel and the tongue guard exposing employees to struck-by hazards. Access to a 220-V electrical panel located in the transfer station area was blocked by a parked front loader machine. Space between the machine and the electrical panel was less than two feet.
