29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) On or about December 2, 2014, and at times prior, Waste Management of Colorado, Inc. did not ensure that the distance between the abrasive wheel and the adjustable tongue guard did not exceed one fourth inch. A Dayton bench grinder was used with a gap of approximately one-half inch between the abrasive wheel and the tongue guard exposing employees to struck-by hazards. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment: (a) On or about December 2, 2014, and at times prior, Waste Management of Colorado, Inc. did not ensure that sufficient access and working space was provided and maintained about electrical equipment to permit ready and safe operation and maintenance of such equipment. Access to a 220-V electrical panel located in the transfer station area was blocked by a parked front loader machine. Space between the machine and the electrical panel was less than two feet. Abatement Note: Abatement certification is not required for this item.
