29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On or about October 30, 2014, at the job site located at 992 E Henri De Tonti in Springdale, Arkansas where employees were installing insulation and standing seam metal roofing, the employees were using a personnel platform mounted on a Gehl Model 553, extendible boom, rough terrain fork lift, that was not approved by the manufacturer for lifting personnel, to access the roof of the building. These conditions exposed the employees to the hazard of falling approximately 22 feet to the ground below.

On or about October 30, 2014, at the job site located at 992 E Henri De Tonti in Springdale, Arkansas, where employees were installing insulation and standing seam metal roofing, the employees were working at the edge of the roofline and were not using any form of fall protection. These conditions exposed the employees to the hazard of falling approximately 22 feet to the ground below.
