29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:     On or about October 30, 2014, at the job site located at 992 E Henri De Tonti in Springdale, Arkansas where employees were installing insulation and standing seam metal roofing, the employees were using a personnel platform mounted on a Gehl Model 553, extendible boom, rough terrain fork lift, that was not approved by the manufacturer for lifting personnel, to access the roof of the building. These conditions exposed the employees to the hazard of falling approximately 22 feet to the ground below.

29 CFR 1926.451(c)(2)(v): Forklifts used to support scaffold platforms did not have the entire platform attached to the forks:  On or about October 30, 2014, at the job site located at 992 E Henri De Tonti in Springdale, Arkansas, where employees were installing insulation and standing seam metal roofing, employees were using a personnel platform, mounted on a Gehl model 553, extendible boom, rough terrain forklift, to access the roof of the building. The platform was not secured to the forks exposing the employees to the hazard of falling approximately 22 feet to the ground below and to the hazard of being struck by a falling platform.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    On or about October 30, 2014, at the job site located at 992 E Henri De Tonti in Springdale, Arkansas, where employees were installing insulation and standing seam metal roofing, the employees were working at the edge of the roofline and were not using any form of fall protection. These conditions exposed the employees to the hazard of falling approximately 22 feet to the ground below.

29 CFR 1926.602(c)(1)(vi): Section 603(e) of American National Standard Institute, B56.1-1969 Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed:    On or about October 30, 2014, at the job site located at 922 E Henri De Tonti, in Springdale, Arkansas, where employees were installing insulation and standing seam metal roofing, the employees used a forklift mounted platform to access the roof. The forklift was left unattended with the platform extended to the roofline, the controls were not neutralized, and the key was left in the ignition. These conditions exposed the employees to the hazard of falling from the platform and to the hazards of being struck by the platform in the event it was inadvertently lowered to the ground.
