29 CFR 1910.132(f)(1): The employer did not provide training to each employee who was required by this section to use PPE: a) Job site, 1232 Pillow Road, Cheswick, PA: On or about December 4, 2014, during tree trimming/removal activities, the employer did not ensure that employees were provided with proper training on fall restraint systems while operating an aerial lift in that an employee wore a belt and was secured to the basket by 2 approximate 5 foot long lanyards, which did not restrain the employee inside the basket.
