On or About December 6, 2014 Employees were utilizing temporary cords to energize portable power tools which did not have a continuous path to ground because the ground pins on several cords were missing.

Employees were engaged in a Gut/Rehab of a commercial mixed use structure. Employees were required to use ladders to move throughout the project. CSHO interviews with employees determined the employer did not provide training to his employees.
