On December 3, 2014, the employer did not conduct training for employees on the new elements of the Globally Harmonized System (GHS) of Classification and labeling of Chemicals.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   a. On December 3, 2014, at the main floor entrance to the Silver Dolphin Bistro restrooms, a portable extension cord was in use that did not have strain relief at the male connector.
