29 CFR 1910.23(a)(5): Every pit and floor opening was not adequately guarded by standard railing or any other effected means. a.) Findlays Tall Timbers Distribution dba Ohio Logistics located at 1800 Production Dr. Findlay, OH 45840; On or about December 9, 2014, the employer did not assure employees were protected from the mechanics pit while working in the Maintenance Building . On October 24, 2014, an employee was exiting the Maintenance Shop when he fell into the stairs of the pit, causing a fractured right leg. b.) Findlays Tall Timbers Distribution dba Ohio Logistics located at 1800 Production Dr. Findlay, OH 45840; On or about December 9, 2014, the employer did not assure employees were protected from the mechanics pit while working on trucks in the Maintenance Building. Employees are required to work on various trucks and are exposed to falls of approximately 4 feet 7 inches.
