29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Jobsite: The employer did not guard the ingoing nip point, created by the drive belt and roller/pulley of the forming machine, exposing the employees to caught by hazards. On or about 12/09/2014, an employee's arm was caught in the roller/pulley while cleaning the above machine, and the employee sustained sustained a broken arm and severe lacerations to the arm from the incident.
