29 CFR 1926.501(b)(1): Employees on walking/working surfaces with an unprotected side and/or edge which is 6 feet or more above a lower level were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:  (a) North Jobsite - On or about December 04, 2014 employees were on the edge of a flat roof preparing to pump wet concrete without fall protection. The employees were exposed to a fall height of 50 feet.

29 CFR 1926.1052(b)(2): Except during stairway construction, foot traffic is prohibited on skeleton metal stairs where permanent treads and/or landings are to be installed at a later date, unless the stairs are fitted with secured temporary treads and landings long enough to cover the entire tread and/or landing area.  (a) North Jobsite - On or about December 04, 2014 foot traffic was allowed on skeleton metal stairs where permanent treads were to be installed at a later date.
