On December 4, 2014, at the lumberyard and warehouse, the employer did not provide forklift training and evaluation for the forklift operators.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   a. On December 3, 2014, in the mechanic shop, live wires of the electrical control switches for the compressor were not enclosed exposing workers to contact electricity and shock hazards.
