When spraying areas are illuminated through glass panels or other transparent materials, the panels did not effectively isolate the spraying area from the area in which the lighting unit was located.    a) Powder Booth, Paint Area, 49 Parry Street, Luzerne, PA - Employees electrostatically applied powder coating materials to metal parts daily within a powder booth illuminated by three (3) of six (6) fluorescent light fixtures that were missing panels. Employees were exposed to burns in the event of a fire or explosion, as observed on or about December 8, 2014.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.107(c)(6): 29 CFR 1910.107(c)(6): Electrical wiring, motors, and other equipment outside of but within twenty (20) feet of any spraying area, and not separated therefrom by partitions, shall not produce sparks under normal operating conditions and did not otherwise conform to the provisions of subpart S of this part for Class I, Division 2 Hazardous Locations:  a) Powder Booth, Paint Area, 49 Parry Street, Luzerne, PA - Employees electrostatically applied powder coating materials to metal parts daily within a powder booth illuminated by fluorescent light fixtures whose power cords were plugged into a metal multi-receptacle electrical box with two (2) dual receptacle outlets mounted to the outside wall (north) of the powder booth that were not approved for use in a Class I, Division 2 hazardous location. Employees were exposed to burns in the event of a fire or explosion, as observed on or about December 8, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:    a) Press Brake Area, 100 Parry Street, Luzerne, PA - Employees handheld 1/4" thick metal parts with their fingers within five-inches of the point-of-operation of two (2) Amada RG-125 Press Brakes, an Amada RG-100 Press Brake and an Amada RG-80 Press Brake during the operating cycle with the gap between the upper and lower tooling components set at 1/2". Employees were exposed to caught-in and caught-between hazards, as observed on December 16, 2014.    b) Press Brake Area, 100 Parry Street, Luzerne, PA - Employees handheld 1/4" thick metal parts with their fingers within five-inches of the point-of-operation of an Amada RG-100 Press Brake during the operating cycle with the gap between the upper and lower tooling components set at 3/4". Employees were exposed to caught-in and caught-between hazards, as observed on December 16, 2014.    In accordance with 29 CFR 1903.19(c), abatement certification and documentation is required for this violation.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed    a) Shipping Department, 49 Parry Street, Luzerne, PA - Employees on a daily basis used six (6) electrical panel switches, adjacent to five (5) unused openings that were not closed, to operate overhead lights. Employees were exposed to contact with an electrical current hazard, on or about December 7, 2014.    In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.
