The equipment for the embosser wheels reached approximately 20" high from the platform with no railing above that and a potential fall to the cement floor of approximately 65".

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): a) On or about 12/11/2014 lockout devices were observed in place in building #1 basement below #1 boiler on gas line valves. The tags being used with the locks did not identify the employee who placed the locks on the system. b) On or about 12/11/2014 locks were observed being used on valve lockout devices for bleach lines on the 2nd level of the Water treatment building. The boiler house locks did not have any identification on them regarding who had placed the locks in place or what the purpose was for the lockout. c) On or about 12/10/2014 locks were applied for #5 PM Broke Chest Pump and Felt Box Pump without the identity of the employee who placed the locks on the disconnects.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 12/9/2014, the VON GAL palletizer did not have the out feed section guarded to prevent entry during operation. b) On or about 12/9/2014, the VON GAL palletizer did not have the pallet in-feed sections guarded to prevent access. c) On or about 12/9/2014, the Tube discharge conveyor on mezzanine, Rapistan conveyor had unguarded ingoing belt nips approximately 6" from the frame. d) On or about 12/9/2014, the CB4 Winder line back stand guard doors were not securely closed. e) On or about 12/10/2014, the #2 Paper machine reel needed additional guarding to protect an ingoing nip point. Top reel drum access was approximately 12" from current guard and 15" from bottom reel drum. f) On or about 12/10/2014, the #2 Paper machine suction pressure roll nip needed guarding. Approximately 24" from green bearing housing to nip point. g) On or about 12/10/2014, the #2 Paper machine supply fan motor openings needed to be guarded which had a rotating fan inside the housing. h) On or about 12/10/2014, the #2 Paper machine winder embossing wheels needed guarding on front and back sides. I) On or about 12/10/2014, the #2 Paper machine winder embossing rolls nip needed guarding. Approximately 4" from bearing block to nip point. Front side labeled calendar rolls. j) On or about 12/10/2014, the #3 Paper machine suction pressure roll nip point was not guarded. The pressure roll had stairs in front of it allowing access to the hazard. k) On or about 12/10/2014, the #5 Pulper has an ingoing nip point of the feeder conveyor that needed guarding. l) On or about 12/10/2014, the #5 Pulper needed guarding to protect operator from being pulled into pulper when feeding bales. m) On or about 12/11/2014, the Flat line facial, on the under side of dryer, the paddles moving past support bar were not guarded creating a caught-in/crushing hazard.. n) On or about 12/11/2014, the VTP Case packer, the case out feed drop area was not guarded.
