On or about December 11, 2014 at 96 Lafayette Drive, Unit 109, Syosset, NY 11791: An employee was elevated by a 40 - Hyster forklift approximately 8 to 9 feet above ground while standing on a wooden pallet that was used for platform without guardrails or personal fall arrest systems in use.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
