Rohr Industries, Inc./Shop Area By Tarped Warehouse Entrance: On or about November 21, 2014, and at times prior thereto, Employees were potentially exposed to fire hazards from the improper use and installation of a Torpedo heater.  The Desa Model torpedo heater was utilized improperly in that it was used less than 10-feet from combustible materials such as a fiberboard drum, plastic tarp, and plastic containers.  In addition, the Torpedo heater was not being operated on a stable surface.  The torpedo heater was being operated off the ground from the top of a fiberboard drum

Rohr Industries, Inc. - On or about 12/9/14, the company did not have a lockout/tagout program consisting of affected and authorized employee training, specific procedures, periodic inspections, and lockout/tagout hardware .  The employees performed work on equipment including but not limited to, Mori Seiki Lathe ZL - 15.  Employees performed servicing and maintenance on equipment which included, but was not limited to, preventative maintenance and repairs.

Rohr Industries, Inc.; Machine shop - On or about, 12/9/14, a CNC lathe (Serial #2619) had a partially unguarded belt/pulley system on the side of it.  The side housing of the machine did not adequately guard the lower part of the belt/pulley system. The pulley/belt system was located 2 feet above floor level.      B.)  Rohr Industries, Inc., Machine Shop - On or about, 12/9/14, an air compressor had an unguarded belt.  The pulley/belt system was located 3 feet above the floor level.

The cord was not listed and labeled to be used in an industrial environment.
