29 CFR 1926.1053(b)(1): The side rails of portable ladder used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder:  On or about December 9, 2014, an employee used a 10-foot A-frame Werner ladder positioned against the edge of the roof on the east side of the house to access the roof, the ladder side rails did not extend at least 3 feet (.9 m) above the roof surface; the employee was exposed to a fall hazard of greater than six feet.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed:  On or about December 9, 2014, an employee used a 10-foot A-frame Werner ladder positioned against the edge of the roof on the east side of the house to access the roof, the A-frame ladder was not designed for that purpose nor was it set properly. The employee using the ladder was exposed to a fall hazard of greater than six feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  On or about December 9, 2014, an employee was observed working on the roof of the newly constructed house without any form of fall protection and was exposed to a fall hazard greater than 6-feet.  Ramirez Heating & Cooling Co. Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313500985, citation number 1, item number 001a, and was affirmed as a final order on October 19, 2011, with respect to a workplace located at 4084 Hueco Landing, El Paso, TX 79938.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  On or about December 9, 2014, the employer did not provide training for the employee performing activities including, but not limited to, working on the roof of the newly constructed house without any form of fall protection and were exposed to a fall hazard greater than 6-feet.   Ramirez Heating & Cooling Co. Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1, citation number 1, item number 001b and was affirmed as a final order on October 19, 2011, with respect to a workplace located at 4084 Hueco Landing, El Paso, TX 79938.
