29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.   On or about December 9, 2014, and times prior thereto, at the dispatch office, the employer stored materials above the office without a load rating or a sign indicating the approved load limit exposing employees to being struck-by or crushed-by hazards.

On or about December 9, 2014, and times prior thereto, inside the body shop, the employer did not ensure that the east and west exit route doors were equipped or identified with an exit sign.  The employees were exposed to entrapment hazards.
