This condition that may expose employees to an inhalation hazards from an accidental release of ammonia.

29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment to maintain its mechanical integrity, was not consistent with applicable manufacturer's recommendations and good engineering practices, or more frequently determined to be necessary by prior operating experience.      Ammonia Refrigeration System: The employer did not inspect or test the safety shutdown systems at least yearly for the Ammonia Refrigeration system and with applicable good engineering practices.

The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation.    Ammonia Refrigeration System, South Engine Room wall: there was a hole and it was outside of acceptable limits.

29 CFR 1910.119(n): Emergency planning and response. The employer did not establish and implement an emergency action plan for the entire plant in accordance with the provisions of 29 CFR 1910.38. In addition, the emergency action plan did not include procedures for handling small releases.     Ammonia Refrigeration System: The employer did not have an emergency action plan that differentiated between small and large ammonia releases. This condition exposes employees to inhalation hazards from an unexpected release of ammonia.
