(a)Manna Pro Products, LLC @ 4545 Madison Street, Denver, CO 80216; On and before December 10th, 2014, the employer failed to ensure that all places of employment where kept clean to the extent that the nature of the worked allowed. Specifically, large amounts of dust and product were allowed to collect and accumulate in the basement of Manna Pro products main production area. The condition exposed employees accessing the basement to heavy concentrations of dust and possible slip and fall conditions.

(a)Manna Pro Products @ LLC 4545 Madison Street, Denver, CO 80216: On and before December 10th, 2014, the exposing employer failed to ensure that all places of employment were kept clean and orderly and in a sanitary condition. Specifically, the employer failed to ensure that the clean up schedule adequately assured that the basement and basement access points were kept free and clear of dust and product. The condition exposed employees accessing and working in the basement to slip and fall hazards from accumulated dust and product on numerous surfaces.

(a)Manna Pro Products, LLC @ 4545 Madison Street, Denver, CO 80216: On and before December 10th, 2014, the exposing employer failed to ensure that offhand grinding machines had adequately sized and spaced work rests. Specifically, two grinders located in the maintenance area did not have adequate work rests to support the work. The condition exposed employees to struck by and caught between hazards while utilizing the grinders to service equipment.
