29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Laundry Room: A relocatable power tap (RPT) was secured via a self locking plastic strip cable fastener zip tie to the electrical conduit and plugged into a junction box. The use of the RPT allowed excessive loads to be connected to the outlet because three 10 amp washers and the laundry soap dispensing system was plugged into the RPT on or about December 10, 2014.

29 CFR 1910.1030(c)(1)(ii)(B): The employer's Exposure Control Plan did not include the schedule and method of implementation for 29 CFR 1910.1030(f), Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up: (a) Establishment: Employees including but not limited to Registered Nurses, Licensed Practical Nurses and housekeeping staff have exposure to blood and other potentially infectious material in the course of their work, but method of implementation for Post-Exposure Evaluation and Follow-up were not established in the Exposure Control Plan, thus potentially exposing employees to bloodborne pathogens, on or about December 10, 2014.
