29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about 12/10/14 located in the warehouse shipping area pole number M-13 and 23, the fire extinguishers were blocked by pallets, not mounted and not readily assessable.

29 CFR 1910.303(g)(1)(iv)(B): Where the working space required by paragraph (g)(1)(i) of this section is doubled, only one entrance to the working space is required; however, the entrance shall be located so that the edge of the entrance nearest the equipment is the minimum clear distance given in Table S-1 away from such equipment. On or about 12/10/14 located in the warehouse receiving area at dock # 19 left side corner, the electrical panel board # CP-28 was blocked with coat racks and a dry erase board being stacked in front and with a clearance of only 13 inch.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"). On or about 12/10/14 located in the warehouse receiving area, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"), while painting in the warehouse was going on-dust masked were issued to employees.
