29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     (a)  Jobsite, 101 South College Street, Washington, PA 15301:  On or about December 9, 2014, an employee was not tied off while he was working from a roof.  The fall hazard to the employee was about 40 feet to the ground.  The pitch of the roof was 6 in 12.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:   (a) Jobsite, 101 South College Street, Washington, PA 15301:  On or about December 9, 2014, there were two instances where two employees were tied off to the same hinged D ring anchor point.  These employees were working on a steep roof (7 in 12 pitch) and were potentially exposed to a fall of about 35 feet to the ground.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Jobsite:  On or about December 9, 2014, an employee was not wearing a protective helmet (hard hat) while he was walking on the ground below other employees who were on the roof and were working with hand tools.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    (a) Jobsite, 101 South College Street, Washington, PA 15301:  On or about December 9, 2014, an employee was not wearing a body harness.   The employee was wearing a rock climbing seat harness and he was tied of in the front of his waste.
