29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Die Cast Machine Area - On or about December 10, 2014, R.C.M. Industries, Inc. did not ensure the point of operation for die cast machine A005 was adequately guarded from having any part of an employee's body in the danger zone during the operating cycle. An employee was injured when retrieving a casting from the machine and the dies closed, crushing the employee's left hand. No abatement certification or documentation is required for this item.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: a) Die Cast Machine Area - On or about December 10, 2014, R.C.M. Industries, Inc. did not ensure hand tools (pliers) used for placing and removing materials into and from die cast machine A005 permitted easy handling without the operator placing a hand in the point of operation. In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
