29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired. a) For the jobsite at 13250 US Highway 301 S, Riverview, FL, as observed on or about 12/10/2014, employees used a Werner extension ladder to access a 10 ft. 4 in. roof. The ladder was missing a cleat exposing employees to a fall hazard.

29 CFR 1926.501(b)(10): 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: a) For the jobsite at 13250 US Highway 301 S, Riverview, FL, as observed on or about 12/10/2014, employees were exposed to fall hazards of 10 ft. 4 in. while working on a 4:12 pitch roof installing roof vents. No fall protection system was not in place. Stay-Dry Roofing of Tampa Bay, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.501(b)(10), which was contained in OSHA inspection number 314565599, citation number 1, item number 1 and was affirmed as a final order on 06/18/2010, with respect to a workplace located at 5120 N. Florida Ave., Tampa, FL 33603.
