29 CFR 1910.252(a)(2)(iii)(A)(1): The employer did not provide fire watchers whenever welding or cutting was performed, where appreciable combustible material in building construction or contents was closer than 35 feet (10.7 m) to the point of operation: Employees cutting and welding vehicle bodies at weld station #3 were exposed to fire hazards. The open gas tanks stored for re-installation back onto the vehicles were stored 7 feet away from welding and cutting areas.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least 1 hour: Employees working on and near the van line were exposed to fire and explosion hazards. The acetylene and oxygen tanks were stored in the same rack.

29 CFR 1910.253(e)(1): Equipment was not installed and used only in the service for which it was approved and as recommended by the manufacturer: Employees working in the van line were exposed to fire hazards in that regulator gauges were not installed and used as recommended by the manufacturer. The gauges were damaged and still in use at the following locations: a) The oxygen/acetylene pressure regulator gauge near the front of the car for the van weld prep station was missing the safety glass in the gauges. b) The oxygen/acetylene pressure regulator gauge near the rear of the car for the van weld prep station was missing the safety glass in both the gauges. The acetylene gauge was missing pressure placard inside the gauge and the needle was missing so that it was not possible to determine what the actual pressure was for the acetylene regulator.
