29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices.    On or about January 14, 2015, the employer did not document that its equipment complies with recognized and generally accepted good engineering practice according to the ASHRAE-15 and IIAR-2 ANSI Standards or equivalent. Examples include, but are not limited to:    a. West end of building - Outside the "Southeast" and "West", engine room doors there were no visual and audible ammonia alarms. Also the meaning of each alarm was not marked by signs near the annunciators.     b. West end of building - Inside the "West", engine room the were no audible ammonia alarms. Also the meaning of each alarm was not marked by signs near the annunciators.     c. Mezzanine engine room - Remote control of the mechanical equipment in the refrigeration machinery room was not provided immediately outside the machinery room door solely for the purpose of shutting down the equipment in an emergency.    d. Mezzanine engine room - Emergency shutdown procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of each engine room, immediately adjacent to each door.    e. Mezzanine engine room - The "North" double doors were not tight fitting, permitting passage of escaping ammonia to other work areas.
