29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap: On or about December 11, 2014 and at times prior thereto, employees working at a residential building were using two extension cords that had insulation damage. Both were used to power circular saws and one saw also had insulation damage to its cord. The employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems: On or about December 11, 2014 and at times prior thereto, employees were observed working on a residential building. One employee was working on the roof without any form of fall protection. Employees were exposed to fall hazards of greater than six feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about December 11, 2014, the foreman climbed off the ladder and placed it so that the employee working on the roof could come off the roof but he didn't extend the sides to three feet above the landing. The employee was exposed to fall hazards of greater than six feet.
