On or about 12/10/2014, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2014.     1.  On 12/09/2014, a recordable injury occurred when an employee received 3 days away from work due to a lower back injury that occurred when an employee was loading a truck.    2.  On 10/07/2014, a recordable injury occurred when an employee received 3 days of restriction or transfer due to a leg strain injury that occurred when an employee was in the loading area.    3.  On 07/29/2014, a recordable injury occurred when an employee received 2 days of restriction or transfer due to a leg strain injury that occurred when an employee was in the parking lot.
